How to View My Forms

This article will show how to view forms in your account.

Instructions:

Step 1:

• Log in to your GotSport account and Click on Forms and Past Forms.

Step 2:

• You can click on the form that you'd like to view.
•  You can also check if your form is completed and the current status.
