How to complete GA/USSSA Participation waiver (Players)

Overview: As a parent of a player, this article will demonstrate the steps to complete the Girls Academy Participation Waiver Form. Found within the players dashboard.

Instructions:

Step 1:

• Log in to your Gotsport parent account and click on the Family button. 

Step 2:

• Select the player you need to complete the form 

Step 3:

• Click on the Player Dashboard

Step 4:

• Click Details next to the Participation Waiver 

Step 5:

Click on Submit New Report 

Confirm your DOB Information, read the waiver

Sign the digital form using your mouse

Click Save
