Step 1:
- Login to your GotSport account. Click here to view a step-by-step process of how to login.
Step 2:
- Typically, users will be accessing the system through their parent's account login, not the player's profile directly. If this is the case, click Account in the top menu bar to find associated players/child. If the player has their own login for their own GotSport account, skip to Step 3.
- Next click on Family in the left navigation bar.
- Click on the specific player name and continue following the instructions below.
Step 3:
- Once in the player's profile, click on the Dashboard tab.
Step 4:
- Locate the organization (player's club) that required forms are needing to be completed for (NTSSA), and then click Details next to NTSSA Competitive Birth Certificate form.
Step 5:
- Click Select next to the player the form is being filled out for.
Step 6:
- Verify and/or modify personal information (if applicable). Then click Save at the bottom of the page to continue.
Step 7:
- Upload player birth certificate and provide signature. Then click Save at the bottom of the page to continue.
Step 8:

Once your page looks like the screenshot below, you have successfully completed and submitted the form to North Texas Soccer Association.
- You will see a green Approved label (example below)
