Creating Medical Release Form

Step One:

Click the "Forms" tab and then click "Forms" in the sub-menu.

Step Two:

Click the "New" form button to create a new form.

Step Three:

Label the form "Medical Release" and click the "Type" dropdown menu and select "Medical Release" as the form type. You also will have the ability to mark it "Required", which will make it a requirement for each registrant to complete as they go through your organization's programs.

Scroll to the bottom and click "Save".
